Damn! I must be pretty lucky. My PPO (Aetna Managed Choice) plan is fully covered by my employer. Dental and vision are extra, but $40/mo. covers those.
I was totally amazed when the only out-of-pocket expense when Steven was born was a single $10 co-pay.

Alas, 2 years ago my PPO, dental, vision, and life benefits cost me $0. Last year $330 I think. This year it would have been $854.
GAs prices are up too, beef prices are up, beer ain't getting any cheaper, and inflation is 1.8%???

and on top of all that, your mama still dresses you funny!! man, its a hard life bigwill 
i have great paid insurance thru my employer, with united health. our co-pay is very low, and my only out of pocket is dental(about $20 bucks a month).
my only problem with united health is that some specialists wont see patients covered by this company. i am not sure why this is, but my son was turned down by some specialists in san antonio when we informed them of our insurance carrier.
