Unless it is a lot worse than what JohnK has just described, it is a frequent thing that I usually refer to as the "space shuttle" syndrome. If lots and lots of top notch engineers cannot make a rocket that can't blow up once in a while, what is the probabily that regular engineers with create something totally perfect?
